On the Navajo Reservation in northeast Arizona, we worked a one week shift--168 hours on duty, 168 hours off duty. I loved it. Like having a one week vacation every other week. We did 911 calls and general transports. The 168 hour shift was great until we pulled back- to- back general transport calls to Albuquerque whick was 9 hours turnaround. We were backed up by the BLS Navajo fire department when out of the area. The local hospital was mindful of our safety, however, and never dispatched us on general transports unless they knew we were rested enough to do it. In other words, a night of 911 calls would buy you 8 hours of uninterupted freedom before undertaking a general transport.

Thank you for all the feedback. I appreciate the information. My department is currently looking at our scheduling structure and I interested in finding a way to maximize efficiency, in a way that keeps everyone happy.
Like you, ncmedic, our shifts generally spill into a lot of overtime and resources and energy are starting to strain.
